中文摘要: 神经介入手术作为一种脑血管疾病微创治疗手段有广阔发展前景。但是,术中需要使用数字减影血管造影(DSA)确定手术导管相对血管的位置,需向患者注射造影剂。DSA对医生和患者产生X射线辐射危害,造影剂会引起相关并发症。本课题拟采用电磁定位技术代替DSA进行手术导管的定位,研究基于电磁跟踪的神经介入手术图像导航关键技术,主要包括血管骨架的自动提取方法研究,用于空间配准及导管路径规划;磁共振血管造影图像(MRA)中血管的自动分割方法研究,用于构建患者个体血管三维模型;空间配准方法研究,建立磁场坐标系、患者坐标系和MRA图像坐标系之间的关系。通过相关研究开发图像导航软件实时描述手术导管端部在血管中的运动。该导航具有实时性,而且不受术中病人移动影响,同时可以减少X射线对医生和病人的辐射危害,降低造影剂并发症发生的几率。该导航对于提高手术效率和效果具有重要作用,对于推动我国神经介入手术发展具有重要意义。

英文摘要: Neurointerventional surgery is a promising way of minimally invasive therapies of the cerebrovascular diseases. During the surgery, digital subtraction angiography (DSA) is used to locate the catheter's tip in the vessel,meanwhile,the contrast is injected into the patient's vessel frequently. The X-ray radiation derived from the DSA is harmful to both the neurosurgeons and the patients, and the contrast may also induce the contrast complications. We plan to locate the catheter's tip using the electromagnetic tracking technique instead of the DSA, and study the key techniques of an electromagnetic tracking based image navigation for the neurointerventional surgery. These key techniques include (1)the method of automatic extraction of the cerebrovascular skeleton, which is used for registration and path planning of the catheter's tip;(2)the mehtod of automatic segmentation of the cerebral vessels from magnetic resonance angiography (MRA) images, which is used to construct a patient-specific 3D cerebrovascular model;(3)the method of registraiton, which is used to establish the relationship among the coordinate systems of the patient, the image and the magnetic field.
