中文摘要: 猝死和心力衰竭是肥厚型心肌病最主要的死亡原因,其主要病理学基础是心肌纤维化。目前研究认为肥厚型心肌病心肌纤维化与心肌微血管功能障碍之间可能存在着关联。如何早期精确定量心肌纤维化程度,以及心肌纤维化与心肌微血管功能障碍是否存在关联是目前亟待解决的课题。本研究拟在7.0T MR扫描仪上采用多模态MRI中的T1mapping、心肌首过灌注及心肌延迟增强对转基因肥厚型心肌病小鼠模型(TnT-R92Q)在心肌肥厚形成过程中多个时间点的心肌纤维化进行评价,确定T1mapping检出心肌纤维化最准确的参数,并应用心脏电影评价其与心功能的关系,同时使用多低b值扩散加权成像和PET检测心肌肥厚形成过程中心肌微血管功能障碍,并与心肌纤维化进行相关对比研究,整个实验以小鼠心肌组织病理学为参考标准,以期早期精确定量检出心肌纤维化,并阐述肥厚型心肌病心肌纤维化及心肌微血管功能障碍的关系,为临床治疗HCM提供新思路。

英文摘要: Sudden cardiac death and heart failure are the main causes of death in hypertrophic cardiomyopathy, and their major pathological basis is myocardial fibrosis. The present findings demonstrate that there may be relationship between microvascular dysfunction and myocardial fibrosis in hypertrophic cardiomyopathy. Now, how to quantify myocardial fibrosis precisely, and how to determine the relationship between myocardial fibrosis and myocardial microvascular dysfunction are the urgent problems. In this study, multi-model MRI will be used at 7.0 T MR scanner. T1mapping sequnece, myocardial first-pass perfusion imaging and myocardial delay enhancement will apply to diagnose and quantify myocardial fibrosis in transgenic mouse model of hypertrophic cardiomyopathy (TnT-R92Q)at different time ponits during myocardial hypertrophy course precisely ,and to determine the most accurate parameter of T1mapping assessing myocardial fibrosis.We also evaluate the relationship between myocardial fibrosis and cardiac function by myocardial cine. Meanwhile, low multi-b-value diffusion weighted imaging and PET were used to evaluate myocardial microvascular dysfunction, and investigate the correlation with myocardial fibrosis, and myocardial histopathology of mouse is considered as reference standard. We hope to quantify myocardial fibrosis precisely at early course, and to determine the relationship between myocardial microvascular dysfunction and myocardial fibrosis in order to provide cardiologists the novel therapy method of patients with hypertrophic cardiomyopathy.
