中文摘要: 儿童失神癫痫(CAE)是一种常见的儿童期癫痫。研究表明:高频神经磁信号(HFNs)在癫痫的发病中可能起重要作用;发作期和发作间期默认网络(DMN)的异常与CAE有关,但DMN功能障碍和常见的CAE神经心理问题(如注意力缺陷)之间关系不清楚。我们前期对14例CAE进行脑磁图(MEG)与弥散张量成像技术(DTI)的初步研究结果发现:在CAE的大脑中存在HFNs,异常HFNs定位在内侧前额叶和楔前叶/后扣带回,这些结构是默认网络的节点;CAE的大脑皮质存在白质完整性的改变。为明确HFNs和白质完整度对于CAE能否成为潜在、新的生物标志,本项目利用MEG在发作间期探测CAE患儿的DMN节点中存在的病理HFNs;通过MEG导航DTI追踪技术评估CAE患儿DMN节点的异常结构连接;明确DMN的功能和结构异常与CAE患儿注意力损害是否有关。本研究的开展将更好的管理CAE患儿并改善其整体生活质量。

英文摘要: Childhood absence epilepsy (CAE) is a common childhood-onset syndrome. Studies suggested a key role of high frequency neural signals (HFNs) in initializing epilepsy. While abnormal default mode network (DMN) during both ictal and interictal periods are believed related to CAE, the relationship between DMN dysfunction and common neuropsychological problems (e.g. attention impairments) in CAE remains unclear.Magnetoencephalography(MEG) and diffusion tensor imaging(DTI) data of 14 children with CAE have presented aberrant HFNs in the medial frontal and precuneus/posterior cingulate cortices, which are nodes in the DMN, and alteration of white matter integrity in extensive brain areas . In order to determine the possibility of neuromagnetic HFNs and white matter integrity taken as biomarkers for study of CAE, in this study MEG was introduced to survey pathological HFNs in DMN nodes of CAE during ictal period, MEG-guided DTI tracking was undertaken to assess abnormal structural connectivity among the nodes of the DMN, and to clarify the correlation between the aberrant functional and structural findings of the DMN and correlate with neuropsychological deficits. This study will contribute to a better clinical management and improvement in the overall quality of life for children with CAE.
